The survey is open to all residents and visitors in the Central Queensland, Wide Bay and Sunshine Coast regions, aged 18 years and over, and all responses are anonymous and confidential. Take the survey to tell us what really makes you feel you’re on the path to better health. Speak up for your chance to win a $100 grocery card!
The My Healthy Community Survey 2024 will only take 10-15 minutes to complete and you could win one of 20 x $100 grocery cards. After you’ve completed the survey, you can enter the draw to win! Terms and conditions apply!
